Treatment
Inrecentyears,rapidadvancesinmedicaltechnology,especiallytheemergenceofanti-angiogenesisdrugs,havegivenmoreoptionsforthetreatmentofdiabeticretinopathy,suchaslasercombinationIntravitrealinjectionofanti-angiogenicdrugswillhaveabettereffectonthecontrolofmacularedema;forexample,incaseswhereneovascularizationhasalreadyoccurred,intravitrealinjectionofanti-angiogenicdrugscanhelptheretreatofneovessels,especiallyinneovascularization.Whenlasertreatmentisnotpossibleontheopticnervehead;insomecaseswithadvanceddiabeticretinopathyandsurgery,youcanalsoinjectdrugsbeforesurgery,whichcanreduceintraoperativebleeding.
Therearetwosituationsthatneedlasertreatment,oneismacularedema.Manystudieshaveshownthatcarefulandweakscatteredlaserphotocoagulationinthemacularareacanreducebloodvesselleakage,therebyreducingedemaandimprovingvision.Second,whenitisfoundthattherearenewbloodvesselsintheretina,ortherearelargeretinalbloodvesselswithoutbloodsupply,atthistime,inordertoshrinkthenewbloodvesselsorpreventthegrowthofnewbloodvessels,so-calledpan-retinallaserphotocoagulationisperformed.
Itisimportanttoemphasizetheimportanceoflaserphotocoagulationinthetreatmentofdiabeticretinopathy.Thecorrectlaserphotocoagulationtreatmentistheprotectorofvisionforpatientswithdiabeticretinopathy!Moreover,thelaserdoesnotjusthititonceorseveraltimes.Forpatientswithdiabeticretinopathy,lasertreatmentmayberequiredatanytimeduringtheregularreviewprocess.Asforhowtoperformlasertreatmentandwhentoperformlasertreatment,followthedoctor’sadvice.Generallyspeaking,pan-retinallaserphotocoagulationwillbedonein3to4times.Itisnotrecommendedtodotoomuchatonetime,toomuchwillcauseaggravationofedema.

Ifthebloodsugarofthediabeticpatientisnotwellcontrolled,andthelasertreatmentisnotperformedintime,theretinopathydevelopstotheproliferativestage,thevitreoushemorrhagedoesnotabsorbformorethan1month,ortheretinahasaproliferativemembrane,andthereisretinaldetachment,whichwillaffectCentralvisionmustbetreatedsurgically.Thepurposeoftheoperationistoclearthebleeding,peelofftheproliferativemembrane,reattachtheretina,andfurtherstrengthenthelaserphotocoagulationtreatment.Twopointsmustbereminded:First,itiscurrentlyunanimouslyrecognizedthattheoperationshouldbeearlyandnotdelayed.Generally,thebleedingcannotbeabsorbedforaboutonemonth,andsurgeryandlasertreatmentshouldbeperformed.Don'twaituntiltheproliferativemembranehasmanynewbloodvesselsandtheretinaldetachmentissolargethattheproliferativemembranecannotberemovedduringtheoperation.Thesecondistorealizethatoncethediabeticretinopathyreachesalargenumberofproliferativemembranes,thecurativeeffectofsurgeryisgenerallyunsatisfactory.Don'texpecttoomuchinthisregard.Therefore,itisemphasizedagain:closereview,follow-up,andearlytreatment.
